Book your appointment with Katelin using her online scheduling system, Jane. Once clicking the teal button you will be redirected to her personal appointment calendar. Please note all appointments do require approval. You can expect to receive confirmation of your appointment within 24 hours of booking. Once approved you will receive an email and link to setup your Jane account and patient portal. In your patient portal you will see attached forms. Please complete the new client intake form, consent forms and any questionnaires a minimum of two business days before your appointment so they can be reviewed before your visit.

Getting ready for your first visit, it's as easy as 1, 2, 3!
- Paperwork [new client intake form & consent forms] after your appointment has been confirmed and your patient portal is setup you will receive an email with pre-consultation paperwork, please complete at least 2 days before your appointment so Katelin has time to review your health history making your fist visit more efficient.
- Supplements, Labs & Medical Records please bring to your visit all lab results, medical records and supplements (list of name, dose, brand is fine too) related to your concerns
- Pen & Paper for taking any notes, I will provide you with a folder
